A presentation missing one of the three is incomplete, and, following reversal cost as the decisive factor, the missing one is usually the reversal cost. But completeness is only one axis. An option can be fully and completely presented and still be the wrong option, because its give-ups violate the constraints. Those are different problems with different fixes.

Two axes: complete versus appropriate

A presentation can be complete and appropriate (the recommendation), complete but inappropriate (a well-presented option that trades away a required gate), or appropriate but incomplete (the right option, but presented without its reversal cost). The exam's favourite configuration puts all three in front of you and asks you to name which is which.
Accurate and consistent is not complete
The trap that makes this evaluate-level is that an incomplete presentation can be internally consistent and factually correct in everything it says. It reads well, its numbers are right, and nothing in it is false. It is incomplete only in the negative sense: it never states the reversal cost. A reviewer scanning for errors finds none, because the missing element is an absence, not a mistake. The discipline is to evaluate against the three-element checklist rather than against a hunt for inaccuracies, because the gap is defined by what is not there.
Incomplete is not inappropriate
The second discipline is not to collapse the two failure modes. An option that is inappropriate for the constraints, one whose give-ups trade away a required audit trail, is not fixed by adding the missing element; it is the wrong option and belongs disqualified, as covered under recommending an option against stated constraints. An option that is appropriate but incompletely presented is fixed by supplying the reversal cost; the option itself is fine. Calling an inappropriate option "just incomplete," or dismissing an appropriate-but-incomplete option as "wrong," both misdiagnose the situation and prescribe the wrong remedy.

The first trap is marking a presentation complete because it is factually correct, without checking that all three elements are present. The exam supplies accurate, consistent presentations that are simply missing the reversal cost, and the credited answer catches the absent element rather than certifying the accuracy.
The second trap is confusing an option that is inappropriate for the constraints with one that is appropriate but incompletely presented. The exam places both in the same set, and rewards diagnosing each correctly: the inappropriate option is the wrong choice, while the incomplete one is the right choice missing an element. Prescribing "add the reversal cost" to an inappropriate option, or "disqualify" to an appropriate but incomplete one, is the error.
