When an outdated or misconfigured custom Skill degrades output, it tends to do so quietly: it produces a slightly off format on every run, consistent enough to look intentional and subtle enough to miss. Because a Skill carries a procedure that runs the same way each time, its defect shows up the same way each time, which is a clue that the Skill, not any single conversation, is the source.
The critical judgement is where to fix it. A degrading Skill is a maintenance problem at the Skill level, and the fix is to re-upload a corrected version. Trying to paper over it with compensating prompt instructions, telling Claude in each conversation to adjust for the Skill's error, does not repair the Skill and leaves the defect in place for everyone and every run that does not include the workaround. The right move is to update the artifact that carries the procedure, not to fight its output conversation by conversation.
What the CCAO-F exam trips candidates on
The exam sets two traps. The first is assuming a custom-uploaded Skill updates itself the way an Anthropic-built Skill does, and never re-uploading a corrected version. The scenario shows a custom Skill left untouched on the expectation it would improve on its own; the credited answer re-uploads a fixed version. Expecting automatic updates for a custom Skill is the tell.
The second is trying to fix an outdated custom Skill by adding compensating prompt instructions instead of updating the Skill itself. The scenario patches the output per conversation; the credited answer repairs the Skill at its source. Both traps reward knowing that custom Skills need manual re-upload and that a Skill-level defect is fixed at the Skill level, not worked around in prompts.

A team's custom-uploaded report-formatting Skill has started producing reports with a slightly wrong section order on every run. One person suggests waiting for it to auto-correct like the Anthropic-built Skills do; another suggests adding a note in each chat telling Claude to reorder the sections. Which is right, and what should actually be done?
Neither suggestion is right, because each misreads how custom Skills version and where the fix belongs.
Waiting for the Skill to auto-correct is the first trap. Only Anthropic-built and organization-provisioned Skills update automatically; a custom-uploaded Skill changes only when someone re-uploads it, so this one will keep producing the wrong section order indefinitely on its own. The consistent, slightly-off format on every run is exactly the fingerprint of an outdated custom Skill.
Adding a per-chat note to reorder the sections is the second trap. It compensates for the defect conversation by conversation without repairing it, so the Skill stays broken for every run that omits the note and for everyone else using it. The correct action is to fix the Skill at the Skill level: correct the section order in the Skill and re-upload the new version. That repairs the procedure at its source, so every future run comes out right without any prompt workaround.
