The Claude Certified Associate - Foundations (CCAO-F) exam treats them together as an understand-level skill because they solve the same problem from two angles: constraints govern what goes into the response and in what register, and output format governs the shape it comes out in.
The reason they matter is economic. Every boundary you leave unstated is a boundary Claude has to guess, and every wrong guess costs you a correction round. A response that is accurate but three times too long, or written in the wrong register, or delivered as prose when you needed a table, is not usable output - it is a starting point for editing. Constraints and output format are how you get a near-final draft instead.

Constraints keep the response inside the lines
Constraints are the boundaries that keep a response usable. The four common kinds are length (how much you want), tone (the register and stance), inclusions (what must appear), and exclusions (what to leave out or avoid). Each one closes off a way the output could technically satisfy the task while still being wrong for your purpose.
Length is the one most often forgotten, and its absence is expensive. A prompt that fixes tone but says nothing about length routinely comes back accurate and far too long, forcing a trimming pass that a single phrase - "under 120 words" - would have prevented. Tone works the same way: unspecified, it defaults to a generic middle register that may not match the audience. Inclusions and exclusions matter when the response must cover certain points or steer clear of others; without them, Claude has no way to know which is which. Constraints are cheap to state and costly to omit.
Output format fixes the shape before you get it
Output format is a separate lever from constraints, and it governs the physical shape of the result: a table, a bulleted list, a short memo, a draft email, a ranked list. Stating it up front is one of the highest-return habits in prompting because the alternative is expensive. If you do not name the shape, Claude picks one, and if the one it picks is not what you needed, you spend an entire extra round asking it to reformat content that was otherwise fine.
The key point the exam draws out is that Claude will not reliably default to your preferred format. There is no house style it knows about; the "obvious" shape in your head is invisible to it. A three-row comparison table, a five-bullet summary, a two-paragraph memo - each is a deliberate choice you state, not something Claude infers from the task. Naming the format is how you collapse a two-round exchange into one.
What the CCAO-F exam trips candidates on
Two traps recur, and both are about a boundary left unstated.
The first is specifying tone but omitting length, then being surprised the draft needs heavy trimming. A scenario fixes the register carefully, leaves length open, and offers a distractor that blames the model for verbosity. The credited reading is that the missing length constraint caused the overrun - add it rather than editing every time.
The second is assuming Claude will default to the requester's preferred output format without it being stated. A scenario expects a table and gets prose, and a distractor treats that as Claude misunderstanding the task. The reliable reading is that the format was never specified, so Claude had nothing to match - the fix is to state the shape up front, which also avoids the extra correction round.
Worked example
A user asks Claude for 'a professional, friendly summary of these five customer complaints.' The result is warm and accurate but runs to eight paragraphs of prose, when the user wanted a short scannable list for a status update. What went wrong and how is the prompt repaired?
The prompt specified one boundary well and left two others open. Tone was handled - "professional, friendly" is a clear constraint, and the output honoured it. But length was never stated, so Claude had no ceiling and produced eight paragraphs. And output format was never stated either: the user pictured a scannable list, but the prompt implied prose, so prose is what came back. Both gaps are the requester's, not the model's - Claude satisfied every boundary it was actually given.
The repair adds the two missing levers explicitly. Constrain the length: "keep it under 120 words." Specify the output format: "as five one-line bullets, one per complaint." Now the prompt states tone, length, and shape, and the first result is usable without a trimming-and-reformatting round. The exam lesson is that accurate-but-unusable output almost always traces to an unstated constraint or format, and the efficient fix is to name it before sending, not to correct it after.
